To be eligible for services you are first assessed by the Aged Care Assessment Team. This is a free assessment and we can assist you with the required referral, which can come from your local doctor, your family or yourself. You can also apply through the My Aged Care website.
The Support at Home program
Government-funded aged care services in your home, helping you remain at home safely. There are four funding types.

How we can help
Our Care Partners will:
- Visit your home and discuss your individual needs, to develop a support plan based on your goals
- Talk to your family, person responsible or friends, if you wish
- Liaise with other services as needed
- Find and implement creative solutions
- Provide ongoing support within the guidelines of the package, to help you remain at home for as long as possible
